| 内容記述 | 学校保健安全法は、学校における健康の保持増進並びに安全の確保が図られることを目的として定められ たものである。勿論、これは食物アレルギーアナフィラキシー緊急対応における救急措置が円滑に行われる ことも意図している。「食物アレルギーの診療の手引き2020」検討委員会では、わが国におけるIgE依存性 の食物アレルギー有症率は諸家の報告を紹介し、乳幼児期から学童期にかけてそれぞれ8%から3%前後とな り次第に増加傾向で、文部科学省も2022（令和4）年から9年ぶりに大規模調査を開始している。2012(平 成24)年12月に東京都調布市立小学校で5年女子児童が食物(給食)アレルギーによるアナフィラキシーショッ クの疑いで給食の3時間後に死亡が確認されるという痛ましい事故があり、それを契機に全国の学校で危機 管理に高い意識を持つようになり緊急対応マニュアルが作られるなど対策が進められた。しかしながら、調 布市立学校の事故を受けその後10年間以上が経過している今「時間の確保」や「教員の関心の低さ」が生じ、 子どもの命を守るために食物アレルギー症状の緊急対応時に求められる行動力を再検証しなければならない 時期に来ていると考えられる。さらに、2018(令和元)年から文部科学省は「教員の働き方改革」実施の推奨、 2020(令和2)年からはコロナ禍に遭遇し、時間的かつ物理的に対面研修を設けることが厳しい情勢が存在し ていることから、これらを踏まえた子どもの命を守るための実効性のある対応を検証する必要がある。そこ で本研究では、学校現場の校長及び養護教諭、栄養教諭、保健体育科教諭に面接調査を行い現況の課題と今 後対応についての意識調査をすることにした。面接調査は九州内地方都市における対応状況を調べるため、 中規模程度の1小学校の校長、養護教諭及び栄養教諭並びに1中学校の校長、養護教諭、保健体育科教諭の6 人で集団討論の形で行った。今回の面接調査では、小・中学校においての食物アレルギー緊急時対応に関す る職員研修の重要性の認識には大きな違いはなく、子どもの進級に絡む事情把握などの情報共有、地域での 学校やその他消防署など外部機関との連携及びICTをも活用した研修の展開の必要性についてそれぞれの学 校や配置教諭の特性の立場から指摘があった。「働き方改革を推進するための関係法律の整備に関する法律」 が2019(令和元)年4月に制定、その各項目が年次計画で順次施行されていることが影響し、職員研修の時間 確保は平時では業務切迫で難しい状況にあるため、児童生徒の長期休暇中の時期に移せば実施できるとの回 答があったが、かつコロナ禍による制限があったとしても、子どもの命に関わる問題であるため職員研修な どの訓練は先送りすることなく、過去に起こった事故を教訓としながら、各種情報共有、地域連携を深め、 対面とICT利用によるハイブリッド形式を導入するなど工夫をして緊張感を伴う効果的な職員研修の改善充 実が求められる。 |

| 内容記述 | The School Health and Safety Act was established with the aim of promoting health and ensuring safety in schools. Naturally, this includes the smooth implementation of emergency measures for food allergy anaphylaxis. The “Guidelines for the Diagnosis and Management of Food Allergies 2020” committee introduced reports on the prevalence of IgE-dependent food allergies in our country, indicating that IgEdependent food allergies gradually increase from around 8% to 3% during infancy and elementary school age. Recognizing this trend, the Ministry of Education, Culture, Sports, Science and Technology initiated a largescale survey in 2022, the first in nine years. Therefore, in this study, interviews were conducted with principals, school nurses, nutritionists, and physical education teachers at the school site to investigate the current challenges and gather awareness regarding future responses. The interview survey was conducted through group discussions with six participants: the principal, school nurse, and nutritionist from one medium-sized elementary school located in the same region of a prefectural city in Kyushu, as well as the principal, school nurse, and physical education teacher from one junior high school. In this interview survey, there was a general recognition of the importance of staff training regarding emergency response to food allergies in elementary and junior high schools. Participants pointed out the need for information sharing, such as understanding the circumstances related to children's grade advancement, collaboration with external organizations such as local schools and fire departments, and the necessity of implementing training that utilizes ICT, based on the characteristics of each school and staff member's placement. The "Act on the Establishment of Related Laws for Promoting Work-Style Reforms" was enacted in April 2019, and each item has been sequentially implemented in annual plans, which has had an impact. Securing time for staff training is difficult during regular periods due to urgent job demands. In response to this, there was a suggestion to conduct the training during the long vacation periods of students, and it was acknowledged that even with the restrictions imposed by the COVID-19 pandemic, since it involves issues related to children's lives, training such as staff training should not be postponed. Instead, efforts should be made to enhance effective staff training with a sense of urgency by drawing lessons from past incidents, promoting various forms of information sharing and community collaboration, and introducing a hybrid format that combines face-to-face and ICT utilization. |
